In older men after the ages of 45 or 50, the prostate gland begins to grow in size. This condition will affect all men however the rate at which the prostate will grow and the symptoms this causes will vary from man to man. Some may not have any complaints in-spite of having a large prostate while others may have typical symptoms like
Getting up at night multiple times to pass urine
Desire to pass urine very urgently
Desire to pass urine multiple times or increased frequency of passing urine
Straining or having to put pressure while passing urine
Poor stream of urine
Starting and stopping while passing urine
Having to wait for long till urine flow starts
A sense that urine is still left inside even after coming back from the washroom.
Not feeling satisfied about emptying urine completely
Burning while passing urine
Dribbling of urine or wetting of underpants after having come back from the washroom.
The prostate gland is present only in males. It is located around the exit gate of the urinary bladder. Any condition that affects the prostate puts pressure on the flow of urine. Symptoms include:
Wanting to pass urine frequently
A strong desire to urgently pass urine
Waking up at night multiple times to pass urine
Having to put pressure to pass urine
Prostate Infection
Infection of prostate gland may occur in all ages of men. Though it is commonly seen in older men who have enlargement of the prostate, or in men who have diabetes or weak immune systems, it may be seen even in younger men where it maybe mistaken for urine infection and produce typical symptoms like burning in the urine and passing urine frequently and with discomfort.
Sometimes if the infections are more serious, there may be fever, inability to pass urine at all (retention of urine), heaviness or pain in the part you sit on, pain or discomfort in passing faeces (stool) or pus formation in the prostate.
Infections need treatment with antibiotics and investigations need to be done to find out why the infection occurred and if it can be prevented from happening again. Pus if formed can be drained through trans rectal ultrasound aspiration.
Prostate Ultrasound
Ultrasound is a crucial investigation in evaluating the prostate. This is done either as part of a routine pelvic ultrasound or transrectal ultrasound (TRUS) or both.

Prostate Biopsy
Biopsy is the process to remove a small bit of the prostate under local anaesthesia. This is done when there may be a doubt about prostate cancer either during examination of the prostate or during some blood tests or family history of prostate cancer.
A systematic 12 or 14 core Trans Rectal Ultrasound (TRUS) guided biopsy is the ideal method and the tissue is sent for histopathological examination (HPE). This facility is available in the Ultrasound unit at our centre.

Prostate cancer is one of the leading cancers affecting men today. It has been linked to a westernized diet and lifestyle but is also seen due to increased detection by easily available blood tests like PSA (Prostate Specific Antigen) and in family history of such cancers.

Tests to detect prostate cancer include an examination by your urologist, blood tests like PSA, biopsy of the prostate, MRI, Bone scan, PSMA PET scan. These seem confusing and too many but not all men need all these tests. Also these are often done sequentially rather than all at once. Only if one test doesn’t give all information is a next test done.
Prostate Cancer treatment
Prostate cancer needs thorough investigations to detect and stage and then depending on the age of the patient, stage of disease, aggressiveness of disease a treatment plan can be formulated. There is a vast amount of data and information on prostate cancer that is available but this needs to be discussed based on the individual persons disease. Vast number of options exist which include just waiting and watching, repeated testing (follow up for any progress) but no active treatment, radiation, hormonal therapy (Androgen Deprivation Therapy) or surgery. Even surgery has multiple options like open, laparoscopic or robotic surgery. However long term data shows that there is no difference in the outcomes between these methods. At the same time, patients need to discuss not only the benefits but also the limitations and problems associated with each of these treatment options.
Just because a treatment method is newly available doesn’t make it the best option for you.
